What to Do If a Life-Threatening Injury Occurs on the Job at FGCU?

In the event of a life-threatening medical emergency, transport the injured employee to the nearest medical facility or call 911 for emergency assistance. After ensuring that an employee receives emergency medical care, AmeriSys, the State of Florida’s medical case management vendor, should also be notified of any work-related medical emergency as soon as possible by calling 1-800-455-2079.

What to Do If a Non-Life Threatening Injury Occurs on the Job at FGCU?

  • AmeriSys provides 24/7/365 claim reporting through their Triage Unit. At the time the claim is reported, the triage nurse will orient the employee to the workers compensation system, and determine the level of medical care required and make the necessary medical referral. 
  • The injured employee and supervisor should immediately contact AmeriSys by calling 1-800-455-2079. Treatment for any non-life threatening work-related injury or illness must be authorized by AmeriSys prior to obtaining medical treatment. The supervisor should be preparred to provide the following information:
    • Name
    • Address
    • Phone number
    • Employee’s Social Security Number
    • Date of birth
    • Date of hire
    • Rate of pay
    • Hours worked/scheduled per week
    • Location code (0195)

  • Once the claim is filed with Amerisys, the employee or the supervisor must then report the injury to the University Environmental Health & Safety Department at 239-590-1126.
  • After the employee is directed for medical care, the claim will be assigned to the nurse case manager and adjuster team who will manage the claim to conclusion.
  • After the initial medical visit, the employee may need follow up visits. The employee must attend all of the scheduled medical appointments. The employees must also maintain contact with their supervisor often or as directed by the supervisor in order to keep him/her informed about the medical progress, on-going treatment, and prognosis for recovery. Remember: absence from work due to a work-related injury or illness will count toward Family and Medical Leave Act (FMLA) entitlement as set forth in the Act.
  • If the injured employee has any problems with the claim or suffers excessive delays in treatment, they should contact the State of Florida’s Division of Workers’ Compensation at: 1-800-342-1741. Other questions can be directed to Human Resources at 239-590-1400.
